Heh I have thats same screenshot in my folder waiting for me to convert it.
The extra buttons come from a UI (user interface) addon. The one I use is called cosmos and it includes lots of other mods too. You can get cosmos from http://www.cosmosui.org
Once you get cosmos, turn on sidebars, second bar, or pop bar. All of them will give you extra buttons. Also, all of the extra buttons can be mapped to keyboard shortcuts if you'd like.

If you use 2nd bar you dont' have many options (it's either on or off). Side bars are either on or off also. Pop bar is configurable and you can decide how many rows and columns of buttons you want. You can also hide buttons like Dajuda has so that the button only shows up if you drag a spell to it. There is also a "show row on mouseover" option that I've used in the past. You can position pop bar anywhere you want on the screen, it has a small circle that you can click and drag around.

Hey!! no shit talking hunters, I'm a Troll hunter and I have a pet Tiger that kicks ass, his name is Cringer (like heman's green tiger lol) and I also have a parrot that does nothing but follow me around.
